Welcome to Method Wellness & Infusion. By scheduling an appointment, receiving services, or using our website, you agree to be bound by these Terms of Service (“Terms”). Please read them carefully.

1. Agreement to Terms

By accessing our website at methodwellnessandinfusion.com or receiving services at our facility, you agree to comply with and be bound by these Terms of Service and our Privacy Policy. If you do not agree to these Terms, please do not use our services.

2. Services Provided

Method Wellness & Infusion provides medical aesthetic and wellness services, including but not limited to:

  • Injectables (Botox, Jeuveau, dermal fillers, PRP)
  • Skin rejuvenation treatments (facials, chemical peels, microneedling, dermaplaning)
  • Body contouring (Emsculpt NEO, EndyMed)
  • Laser treatments (hair removal, tattoo removal)
  • IV therapy and vitamin injections
  • Medical weight loss and peptide therapy

All services are provided by licensed medical professionals. Services are subject to availability and may be modified or discontinued at any time without notice.

3. Appointments and Scheduling

3.1 Booking Appointments

Appointments can be scheduled:

  • Online through our booking system (Boulevard)
  • By phone at 480-878-0720
  • In person at our facility

3.2 Appointment Confirmation

You will receive a confirmation email and/or text message after booking. If you do not receive confirmation within 24 hours, please contact us to verify your appointment.

3.3 Arrival Time

Please arrive 10-15 minutes before your scheduled appointment time to complete any necessary paperwork. Late arrivals may result in shortened treatment time or rescheduling.

3.4 First-Time Patients

New patients must complete a medical intake form before their first appointment. This can be done online or in person. Please allow extra time for consultation.

4. Cancellation and No-Show Policy

⚠️ IMPORTANT: Please read our cancellation policy carefully to avoid fees.

4.1 Cancellation Notice Required

We require 24 hours advance notice for cancellations or rescheduling. Cancellations can be made:

  • Through your online booking confirmation link
  • By phone at 480-878-0720
  • By email (if 24+ hours in advance)

4.2 Late Cancellation Fee

Cancellations made with less than 24 hours notice will be charged:

  • $100 fee

4.3 No-Show Fee

Failure to show up for a scheduled appointment without prior notice will result in:

  • $100 fee

4.4 Exceptions

Late cancellation and no-show fees may be waived in cases of:

  • Medical emergencies (documentation may be required)
  • Severe weather conditions
  • Other extraordinary circumstances at our discretion

4.5 Repeat Cancellations

Patients with multiple late cancellations or no-shows may be required to:

  • Provide a credit card deposit when booking future appointments
  • Prepay for services
  • Be restricted from online booking

5. Payment Terms

5.1 Payment Methods Accepted

We accept the following forms of payment:

  • Credit cards (Visa, Mastercard, American Express, Discover)
  • Debit cards
  • HSA/FSA cards
  • Cash
  • CareCredit and other healthcare financing (subject to approval)

5.2 Payment Due at Time of Service

Payment is due in full at the time services are rendered unless other arrangements have been made in advance.

5.3 Pricing

Prices are subject to change without notice. The price quoted at the time of booking or consultation is the price you will pay, even if our pricing changes before your appointment.

5.4 Package Deals and Memberships

  • Package deals must be paid in full at time of purchase unless financing is arranged
  • Package treatments must be used within 12 months of purchase
  • Packages are non-transferable and non-refundable
  • Unused treatments do not expire but may not be refunded

5.5 Gratuities

Gratuities are appreciated but not required. If you wish to tip your provider, you may do so in cash or add it to your credit card payment.

6. Refund and Return Policy

6.1 Service Refunds

Due to the nature of medical aesthetic services, refunds are generally not provided once a service has been performed. However, we may offer:

  • Complimentary follow-up appointments to address concerns
  • Alternative treatments at no additional charge
  • Partial refunds in cases of provider error or facility issues

6.2 Product Returns

Retail products (skincare, supplements) may be returned within 30 days if:

  • Product is unopened and in original packaging
  • Receipt is provided
  • Product is not expired

Opened products cannot be returned for sanitary reasons.

6.3 Prepaid Services

Prepaid services and packages are non-refundable but may be:

  • Transferred to other services of equal or greater value
  • Used for future appointments (no expiration)
  • Transferred to an immediate family member with written consent

7. Medical Disclaimers and Acknowledgments

7.1 Not a Substitute for Medical Care

Our services are cosmetic and wellness-focused. They are not a substitute for diagnosis, treatment, or advice from a licensed physician. Always consult your doctor about medical conditions.

7.2 Results May Vary

Individual results vary based on factors including:

  • Age, skin type, and overall health
  • Adherence to pre- and post-treatment instructions
  • Lifestyle factors (smoking, sun exposure, diet, exercise)
  • Underlying medical conditions

We cannot guarantee specific results.

7.3 Risks and Side Effects

All medical aesthetic procedures carry some risk. Common side effects may include:

  • Temporary redness, swelling, or bruising
  • Tenderness or discomfort
  • Temporary changes in skin pigmentation
  • Allergic reactions (rare)

More serious complications are rare but possible. Your provider will discuss specific risks during your consultation.

7.4 Informed Consent

You will be asked to sign an informed consent form before each treatment. This form explains:

  • The procedure and expected results
  • Potential risks and complications
  • Alternative treatments available
  • Pre- and post-treatment instructions

By signing, you acknowledge you understand and accept these risks.

7.5 Medical Conditions

You must disclose all medical conditions, medications, and allergies. Failure to do so may result in:

  • Adverse reactions
  • Complications
  • Unsatisfactory results
  • Refusal of service

7.6 Pregnancy and Breastfeeding

Most treatments are not recommended during pregnancy or breastfeeding. Please inform us if you are pregnant, trying to conceive, or breastfeeding.

8. Photography and Marketing

8.1 Before/After Photos

We may request before and after photos for your medical record and treatment documentation. These photos are kept confidential unless you provide written consent for their use in marketing.

8.2 Marketing Use

If you consent to the use of your photos, testimonials, or reviews in our marketing materials:

  • Your identity may or may not be disclosed (your choice)
  • Photos may be used on our website, social media, and printed materials
  • You may revoke consent at any time in writing
  • You will not receive compensation unless separately agreed

9. Behavior and Conduct

We strive to provide a professional, respectful environment. We reserve the right to refuse service or terminate the patient relationship if:

  • You are disrespectful, abusive, or threatening to staff
  • You arrive intoxicated or under the influence of drugs
  • You repeatedly fail to follow pre- or post-treatment instructions
  • You repeatedly miss appointments without notice
  • You engage in behavior that disrupts our facility

10. Minors

Patients under 18 years of age must:

  • Be accompanied by a parent or legal guardian
  • Have parental consent for all treatments
  • Have parent/guardian present during consultation and treatment

Some treatments may not be appropriate for minors and will be offered at provider discretion.

11. Limitation of Liability

To the fullest extent permitted by law:

  • We are not liable for indirect, incidental, or consequential damages
  • Our total liability for any claim shall not exceed the amount you paid for the service
  • We are not responsible for results from failure to follow instructions
  • We are not liable for complications from undisclosed medical conditions

Arizona law may limit our ability to disclaim certain warranties or liabilities.

12. Intellectual Property

All content on our website, including text, images, logos, and videos, is the property of Method Wellness & Infusion or its licensors and is protected by copyright laws. You may not:

  • Copy, reproduce, or distribute our content without permission
  • Use our trademarks or logos without written consent
  • Scrape or harvest data from our website

13. Website Use

13.1 Account Security

If you create an account for online booking:

  • You are responsible for maintaining password confidentiality
  • You are responsible for all activity under your account
  • You must notify us immediately of any unauthorized access

13.2 Prohibited Uses

You may not use our website to:

  • Violate any laws or regulations
  • Transmit viruses or malicious code
  • Harass, abuse, or harm others
  • Impersonate any person or entity
  • Interfere with website functionality

14. Third-Party Services

  • Boulevard (appointment booking)
  • Payment processors
  • Email marketing platforms

These services have their own terms and privacy policies. We are not responsible for their practices or any issues arising from their use.

15. Modifications to Terms

We reserve the right to modify these Terms at any time. Changes will be effective immediately upon posting to our website. Continued use of our services after changes constitutes acceptance of the modified Terms.

We will make reasonable efforts to notify you of material changes via:

16. Governing Law and Dispute Resolution

16.1 Governing Law

These Terms are governed by the laws of the State of Arizona, without regard to conflict of law principles.

16.2 Jurisdiction

Any legal action arising from these Terms must be filed in the state or federal courts located in Maricopa County, Arizona.

16.3 Dispute Resolution

Before filing any legal action, you agree to:

16.4 Class Action Waiver

You agree that any claims will be brought individually and not as part of a class action or collective action.

These Terms of Service, together with our Privacy Policy and any signed consent forms, constitute the entire agreement between you and Method Wellness & Infusion regarding our services.
